Credentialing Associate

SUMMARY OF DUTIES: Verifies credentials and processes the information of the provider’s intake application. Attend Committee Meeting and Board of Directors Meeting. Ensure the initial credentialing and re-credentialing process is accurate and complete. Ensure compliance with delegated credentialing policy and procedures. Schedule provider’s training as needed. Take and complete minutes for Credentialing Committee Meeting. Completion of provider’s applications on an annual basis. Other duties as assigned. EDUCATION: Associate’s Degree in Business Administration or related field. EXPERIENCE: 3 years of Managed Care experience involving credentialing process.  SKILLS: Must be bilingual (English/Spanish).  Excellent customer service skills.  Excellent organizational and interpersonal skills. Proficient with Microsoft Excel, Access, and Word.

WIC Nutritionist CPA3 (WIC)

Under the supervision of the WIC Program Director. Will be responsible for developing educational materials.  CPA3 will be responsible for assessment of nutritional needs, education and counseling of the Women, Infants, and Children supplemental Food Program (WIC).  Other duties as assigned.   EDUCATION: BS Nutrition or higher; in related field.  Two years of experience in the field of Community Nutrition.  EXPERIENCE: Previous work with women, infants, and children.  Work with WIC program preferred.  Minimum of 2 years of experience in the field of community nutrition.  SKILLS:  Excellent customer service skills.  Knowledge of current nutrition issues.  Basic knowledge of computer and programs. Bilingual (English/Spanish) preferred.

Case Manager (PDS)-Temp

Ensure patients access to needed services and community resources by arranging transportation. Provide comprehensive supportive services. Open new patients’ medical records and conduct new patients screening. Coordinate linkages and referrals to other department programs or to external service providers, integrating other agency or service activities into the Treatment Plan. Complete forms, letters, applications for patients as needed. Adhere to National Patient Safety Goals. Participate in case conferences and quality improvement activities. Other duties as assigned. EDUCATION: Associates’ degree required.  Bachelor’s degree preferred.  EXPERIENCE: 1-2 years experience in a Medical setting.  SKILLS:  Strong written, organizational, and communication skills.  Computer knowledge.  Bilingual (Spanish/English) required; read and write both languages.
