Family Physician (Plaza del Sol)
Examines or conducts tests on patients to provide information on medical condition. Analyzes records, reports, test results, or examination information to diagnose medical condition of patient. Prescribes or administers treatment, therapy, medication, vaccination, and other specialized medical care to treat or prevent illness, disease, or injury. Explains procedures and discusses test results on prescribed treatments with patients. Complete and submit appropriate progress reports to CMO. Advise patients and community concerning diet, activity, hygiene, and disease prevention. Provides 24-hour on-call service on a rotating weekly basis. Perform other job-related duties as required by departmental needs/demands. Graduate of an accredited Medicine School and completion of a postgraduate training in an accredited postgraduate program. Valid NYS license; board certified or eligible. Must pass appropriate skills/tests/competency. Bilingual Spanish/English preferred.
Director of Social Services
Coordinates the Social Service program across all UHP sites, which includes the provision of social service entitlements, referrals to community resources, patient advocacy, brief counseling, crisis intervention, abuse and neglect oversight, staff training and case management. Oversees the following programs: Geriatric, HIV, Hep C, Prenatal, Psycho-social and New patient intake. Provide direct Social Services to patients and participants in the organizations Quality Improvement program. Oversee quality of work of all Social Service associates.
Geriatric Case Manager (Temp, F/T)
Provides case management services to patients as assigned for the following areas: Geriatric population, IDC, Pediatrics, Psychiatry, etc. Provide assessment, referrals, advocacy, entitlements, and supportive counseling. Coordinate linkages and referrals to other department programs or to external service providers, integrating other agency or service activities into the Treatment Plan. Complete forms, letters, applications for patients as needed. Adhere to National Patient Safety Goals. Participate in case conferences and quality improvement initiations. Other duties as assigned.
WIC Program Assistant (Plaza del Sol)
Responsible for providing administrative support to the WIC Program. Provides respectful supportive collaboration to promote teamwork for achieving patient satisfaction and organizational goals relative to services. Determines program applicants’ and participants’ eligibility for WIC Program. Roster in participants for appointments. Work closely with WIC CPAs; Conduct follow ups and no shows; data entry, filing, answering phones, etc. Other duties as assigned. HS Diploma/GED required. Experience with children and/or pregnant women; computer literate. Other experience in medical field and clerical experience a plus. Strong written, organizational, communication, and customer service skills. Bilingual (English/Spanish).
WIC Nutrition Assistant
Perform nutrition screening test according to standard procedures including height and weight measurements and hemoglobin testing. Updates and maintains participants’ computer files along with data entry. Other duties as required. Conducts basic group nutrition education. High School/GED, college credits preferred. One (1) year of relevant clerical support work experience.
LPN (Temporary, full-time) Adolescent/Pediatrics Department
Nursing care, organizing and assisting in patient care. Patient evaluations and monitoring, administering medications and immunizations to age specific patients, provide patient education. Administering asthma treatments, as well as monitoring other duties as assigned. New York State Practical Nurse license. 1-2 years practical nursing experience, Computer literate, Excellent communication and interpersonal skills.
QDirector of Health Education (Training and Development/Learning Center)
Under the direction of the Chief Executive Officer, plans, coordinates and directs health educator, case managers and community health educators in providing health education programs for the organization. Inherent in this role are the knowledge, skills and attitudes specific to health promotion and disease prevention. The DHE’s role includes consultant, educator, case manager, community developer, facilitator, advocate, counselor, communicator, coordinator, collaborator, researcher and evaluator, social marketer and policy formulator. MPH - Community Health Education. 5-6 years experience in curriculum development, training, outreach, community education. Bilingual, Spanish preferably.
Nutritionist
Provide medical nutrition therapy to all age groups, in particular to OB and Pediatrics patients. Conduct food demonstrations. BS in Nutrition and Dietetics, CDN/RD preferred. Prior nutrition counseling preferred.

Installation of desktop computers, including OS install/ configuration; Windows Active Directory administration (adding users, home/profile folders, Outlook/Exchange email configuration) in a multi-site environment; Installation/Troubleshooting MS Office Suite 2000/2003/2007; configure and troubleshoot network connectivity of computers, printers, and switches. Troubleshoot PC, Network, printer and various applications. Adult Patient Coordinator
Primary duties and responsibilities: Responsible for the overall Adult Medicine departmental operations including registration, patient flow, discharge, appointments, and referrals. This includes both Adult Medicine and Walk-In Clinics. Performs and oversees payroll functions for all departmental associates. Prepares monthly schedules for all associates. Assures appropriate departmental staffing and coverage at all times. Facilitates and oversees the process of the new patients including the coordination of new patients from the 2nd floor to Walk-In and/or Adult Medicine. Other duties as assigned. Bachelors Degree; RN NY State license. Prior experience in an outpatient clinical/community health center setting. Good organizational skills, computer literate (Microsoft Word/Excel). Referral I Call Center Nurse Manager
Primary duties and responsibilities: In consultation with the UHP provider staff, act as a clinical advisor to the Referrals and Call Center. Document all decisions and instructions in the patient’s electronic medical record. Work with Risk Management and Safety Committee based on recommendations for any Unexpected Events. Present date and case management to senior leaders as needed. Other duties as assigned. Graduate of an approved school of nursing with a BSN or related health care field. At least three years of professional nursing experience in an outpatient setting. NYS License as a registered professional nurse. Ability to communicate professionally with all levels of associates. Bilingual (English/Spanish) preferred.
