Community Health Worker

Department-Community Health

Conduct home visits and provides service coordination to (25-30) pregnant and postpartum high risk women. Provide health education through workshops/educational sessions on breastfeeding, family planning, SIDS, smoking cessation and others. Conduct outreach and provide referrals. Bilingual (Spanish/English) required. High School Diploma or GED, and Some College credits or Associates Degree preferred. Background in Public Health especially in child birth education; must be able to conduct workshops and/or presentations. Interviewing skills a plus. Applicant should be well versed in providing referrals to clients and to various support programs. Detail oriented, organized and excellent customer service skills. Excellent reading and writing skills are needed. Ability to work in a team atmosphere; must be able to work flexible hours; should be responsible, and self motivated.

 

Telephone Support Associate

Department-IACH

Provide telephone support to patients to assure appropriate utilization of medical and support services; assist patient in obtaining appropriate health care services (mammograms, attendance at diabetes classes, etc. Self motivated and responsible. Bilingual (Spanish/English) preferred. High School Diploma or GED, and Medical Assistant Certificate. A minimum of 1-3 years experience in Medical Assistant or related position in health care preferred. Detail oriented, organized and compassionate with others. Excellent customer service skills. Computer proficient in MS Word, Excel and ECLO.

 

WIC Chief Nutritionist

Department-WIC

Determines eligibility of applicants/participants, enrolls eligible participants, conducts nutrition in services, promotes nutrition, trains new staff, trouble shoots nutrition problems, other duties as assigned. Prior WIC Nutrition experience. Work well with others/loves children. BS in Nutrition. A minimum of three years WIC experience, or MS in Nutrition, and prior work experience in related health program. Extensive experience and judgment to plan and accomplish goals.

 

Registered Nurse

Department-Plaza Del Sol

Provide nursing care by planning, organizing, and assisting in patient care; provide triage and vaccines and immunization services to age-specific patients, and the ability to follow universal precaution. Familiar with a variety of the health field’s concepts and practices. Flexible schedule. Bilingual (Spanish/English) required. Current NYS license and Registration. Graduate from an accredited RN School. A minimum of two year of experience in nursing including documentation and injections. Extensive experience and judgment to plan and accomplish goals.

 

Medical Assistant

Department-Plaza Del Sol

Responsible for providing patient services and clinical support to Provider by performing thefollowing duties: prepares charts for patient’s visits, takes vitals, collects specimens from patients, draw blood, and perform EKG’s etc. Detail oriented. Bilingual (Spanish/English) required. Flexible schedule is available. High School Diploma or GED. Medical Assistant certificate, EKG andPhlebotomy certified. A minimum of six-(6) month related work experience. Excellent communication skills and customer service skills.

Accounts Payable Clerk

Department-Finance

Reviews and match invoices to P.O. and receiving documents; key enter vouchers; prepare checks; reconcile vendor statements; code expenses per chart of accounts. Solomin software experience required. High School Diploma, Associates Degree preferred. Minimum of three years experience as an A/P Clerk.

 

Medical Records Clerk

Department- Medical Records

Responsible for release of information; maintaining logs; liaison between Staff / patients and warehouse; record request; scanning and QA. High School Diploma / GED. Excellent communication skills and customer service skills. Computer proficient in Medical Manager, MS Word, and Excel.

 

Case Manager

Department-Behavioral Health

Case Manager for the Depression Collaborative; screen and provide case management services to patients; outreach; provide health education; provide basic social services assistance to patients; work closely with the medical assistants and providers in order to facilitate the depression services. Self motivated, responsible, and proactive. Familiar with a variety of mental health concepts, practices and procedures in working with the population/setting. Bilingual (Spanish/English) required. High School Diploma/GED, College degree preferred.

Bilingual Spanish/English for all position preferred.
