HISTORY
- Founded in 1974 by Dr. Richard Izquierdo, a local family practitioner who has been practicing in the community since 1962, as a not-for-profit, Article 28 Diagnostic and Treatment Center by the New York State Department of Health
- Federal Qualified Health Center (FQHC) designation obtained in 1999
- Accredited by the Joint Commission on Accreditation of Health Care Organizations (JCAHO) in 2003; reaccredited in 2006
- Relocated to new, $14 million state-of the art facility in 2001
- Currently there are over 27,000 users and 145,000 patient visits
- 380 staff members including 71 medical providers
