Institute for Healthcare Improvement:

Conducts quality improvement programs using a collaborative model for asthma and other diseases and health care system problems such as waits and delays. www.ihi.org

National Initiative for Children's Healthcare Quality:

A branch of IHI that focuses on children's health. NICHQ has conducted 10 collaboratives to improve pediatric asthma care. www.nichq.org

CDC Program for Translation and Dissemination of Effective Interventions:

This site contains lists programs for asthmas that have been translated to make them easily available. It also contains listings of other programs that have been evaluated and found effective, but not yet translated. Go to www.cdc.gov/asthma and click on Effective Interventions.

Asthma and Allergy Foundation of America:

This organization distributes the Asthma Care Training for Kids (ACT), Wee Wheezers, and You Can Control Asthma Programs. 1-800-7-ASTHMA (1-800-727-8462)

American Lung Association:

This organization distributes the Open airways for Schools program as well as Tools for Schools. 1-800-LUNG USA (1-800-586-4872)

University of Michigan School of Public Health:

National Institute for Allergy and Infectious Diseases:

This agency distributes the Guide for Helping Children with Asthma from the Inner City Asthma Study. 301-496-7353

National Heart, Lung, and Blood Institute:

National Asthma Education and prevention Program. This website lists many asthma resources, including treatment guidelines, educational materials, and the model asthma management program. www.nhlbi.nih.gov

Children's Defense Fund:

This organization has many programs to improve children's health including initiatives to enroll families in the Children's Health Insurance Program. www.childrensdefense.org. Allies Against Asthma:

Aida Fontanez

"My name is Aída Fontanez, and I have been a patient and volunteer at Urban Health Plan, Inc. for eighteen years. Based on my own experience, I believe this health center meets the necessary requirements that make it possible to improve the quality of life to our community---doctors, specialists, innovative programs, medical equipment for diagnoses and its bilingual personnel, make you feel as if you are part of a family. Also, the trust and communication between doctor and patient gives us the understanding to better take care of our health.
