November 10, 2023 — Bronx, NY – Urban Health Plan (UHP), a network of community health centers located in the Bronx, Central Harlem, and Northern Queens, is among seven New York State health centers to receive a 2022 Gold Health Center Quality Leader Badge, it was announced by the Health Resources and Services Administration (HRSA).
As a result, UHP has the distinction of being among the top 10% of health centers in the country providing high-quality care for patients in underserved communities. The award is part of HRSA’s annual Community Health Center Quality Recognition (CHQR) badges, based on Uniform Data System (UDS) information.
Urban Health Plan also received the Advancing Health Information Technology (HIT) for Quality Badge which recognizes health centers that meet all criteria to optimize HIT services. This includes adopting an electronic health record (EHR) system, offering telehealth services, exchanging clinical information electronically with key providers health care settings, engaging patients through health IT, and collecting data on patient social risk factors.
