Program
When we call ourselves a patient-centered medical home, we mean that we provide coordinated and integrated primary and preventive health care services, along with a host of ancillary and support services.
Our services are complemented by a full spectrum of health improvement programs and initiatives, including an HIV/AIDS program, a nationally acclaimed asthma management program, the Canyon Ranch Health and Wellness Center at Urban Health Plan, and a Women, Infants and Children (WIC) program. These programs support our clinical care, ensuring that patients receive comprehensive, well-managed services.
