Bronx, New York, June 17, 2025 — Today, Urban Health Plan (UHP), one of the largest federally qualified health centers in New York State, welcomed local residents and patients to tour its newest site, Plaza San Juan Community Health Center, at 1095 Southern Boulevard. With the completion of construction, the state-of the art, 52,000 square foot health care facility, expands access to quality primary and subspecialty health care to its patients. The four-story health center will also bring economic development and environmental renewal to the area.
“This new health center advances our mission to improve health and quality of life by increasing community access to primary care, critical subspecialty care services, and innovative disease prevention programs,” said Paloma Hernandez, President and CEO of Urban Health Plan.
Plaza San Juan was built on a former Brownfield site, previously occupied by multiple fire-damaged businesses. The project required extensive and costly soil and water remediation. “This would not have been possible without the support of Councilmember Rafael Salamanca Jr. and State Senator Luis Sepulveda,” said Hernandez. “Thanks to their funding and advocacy, we’ve brought Plaza San Juan to life and expanded subspecialty care services, which can be difficult for residents of our community to access,” said Hernandez.

The project has received capital funding from the New York City Council, the New York State Senate and the New York State Department of Health.
“Growing up in the South Bronx, I experienced firsthand how much my community negatively suffered due to health inequalities,” said Council Member Rafael Salamanca, Jr. “It is for this reason that I have dedicated my time as an elected official to championing health centers and programs, like Urban Health Plan, who are committed to closing this gap and ensuring that Bronxites receive quality healthcare. In support of their fearless dedication to the well-being of my community, I was proud to allocate $20,332,400 towards funding this audacious project — Plaza San Juan Community Health Center — to ensure that the Bronx is no longer left behind or forgotten in receiving adequate medical care.”

Plaza San Juan Community Health Center houses 36 exam rooms and offers primary care and specialty services, including:

- Cardiology
- Endocrinology
- Gastroenterology
- Neurology
- Nephrology
- Optometry
- Podiatry
- Rheumatology
- Pulmonology/Biologics Clinic
- On-site laboratory
It features state-of-the-art equipment to facilitate and improve patient care and wait times. Support services, including nutrition, case management, and social services, are also offered. The site is expected to open in the fall of 2025. It joins UHP’s network of 12 health centers, two outpatient mental health sites, 12 school-based health centers, and several other support services and administrative sites.
