Dr. Acklema Mohammad is a distinguished pediatrician and the Chair of Pediatrics at Urban Health Plan. She began her journey at UHP in 1974, and over the decades, she has cared for multiple generations of families, establishing herself as a trusted figure in the community.
Dr. Mohammad has led transformative initiatives aimed at improving pediatric health outcomes. She created UHP’s award-winning asthma management programs, which have gained national recognition from the Environmental Protection Agency. Additionally, she established a Reach Out and Read program at Urban Health Plan, promoting early childhood literacy by incorporating book distribution and reading guidance into pediatric visits. This initiative empowers families to support their children’s development from the very beginning. Dr Mohammad also played a crucial role in developing Quick Care Kids, Urban Health Plan’s urgent care clinic for children.
Dr. Mohammad’s leadership and advocacy have earned her numerous honors, including the Dr. Walter Rivas Leadership Award, the Outstanding Patient Satisfaction Award, and the National Exemplary Award for the Management of Asthma.
Alyssa Jang is a Certified Pediatric Nurse Practitioner specializing in Primary Care. She earned her Bachelor of Arts degree from Wellesley College and a Master’s degree in public health with a concentration in maternal and child health from Boston University. After gaining experience working with children with Type 1 diabetes in the Bronx, Alyssa completed a Master of Science in Nursing at Yale University. She is committed to fostering an environment where patients feel heard and valued, with a focus on improving health outcomes through equitable healthcare practices.
Dr. Anyelina De La Cruz, Pediatrician
Dr. Anyelina De La Cruz is a board-certified pediatrician with over 17 years of experience in providing compassionate and comprehensive care to children of all ages. She earned her medical degree from Universidad Católica Nordestana in 2008 and completed her pediatric residency at Lincoln Medical Center in New York. Since 2017, Dr. De La Cruz has been a dedicated member of the Urban Health Plan (UHP) team, serving the Bronx community.
At UHP, Dr. De La Cruz is committed to delivering personalized, evidence-based care with a strong focus on preventive medicine and early intervention to promote healthy development in children. Her approach emphasizes building strong relationships with families and ensuring each child receives tailored support to meet their unique health needs. She is well-known for her empathetic and patient-centered approach to pediatric care.
Dr. Cynthia Joseph, DO, is a board-certified pediatrician with over 20 years of experience, dedicated to providing compassionate and comprehensive care to children in the Bronx community. She earned her medical degree from the New York Institute of Technology College of Osteopathic Medicine in 2005 and completed her pediatric residency at Maimonides Medical Center/Infants and Children’s Hospital of Brooklyn from 2005 to 2008.
Since joining Urban Health Plan, Dr. Joseph has been committed to ensuring that all children receive the highest standard of healthcare. She is known for her patient-centered approach, taking the time to answer questions and provide follow-up care as needed.
Dr. Joseph’s dedication to children’s health and well-being has made her a trusted provider in the community.
Dr. Laura E. Toner is a board-certified pediatrician with over 25 years of experience. She obtained her medical degree from New York Medical College in 1998 and completed her pediatric residency at Maria Fareri Children’s Hospital at Westchester Medical Center, where she also served as Chief Resident until 2002.
Fluent in both English and Spanish, Dr. Toner is dedicated to providing compassionate and comprehensive care to children and adolescents. She is a Fellow of the American Academy of Pediatrics and has been recognized for her commitment to patient-centered care, having received the National Committee for Quality Assurance’s Physician Practice Connections Recognition Program and Patient-Centered Medical Home Recognition Program awards from 2011 to 2013.
In addition to her clinical work, Dr. Toner is actively involved in community health initiatives and continues to advocate for the well-being of her patients and their families
Mariel Owen-Simon, MSN, CPNP-PC
Mariel Owen-Simon, MSN, CPNP-PC, is a board-certified pediatric nurse practitioner at Urban Health Plan’s El Nuevo San Juan Health Center in the Bronx, New York. She earned her Master of Science in Nursing from the University of Pennsylvania in 2020, where she specialized in pediatric primary care. Since joining Urban Health Plan, she has been committed to providing compassionate, family-centered care to children and adolescents in the surrounding community.
Ms. Owen-Simon is passionate about building strong, trusting relationships with her patients and their families. She emphasizes preventive care, early intervention, and health education to support the long-term well-being of the children she serves. Matthew Hwang, MD., Pediatrician
Dr. Matthew Hwang, MD, is a dedicated pediatrician at Urban Health Plan in the Bronx, New York. He earned his medical degree from the University of Illinois College of Medicine in 2021 and is currently completing his pediatric residency at Jacobi Medical Center/Albert Einstein College of Medicine, where he has been serving since 2021.
Prior to medical school, Dr. Hwang gained valuable experience working as an emergency room technician, which deepened his commitment to providing comprehensive and empathetic care. His clinical interests encompass holistic approaches to pediatric health, emphasizing the integration of psychological well-being, nutrition, and harm reduction strategies. Dr. Hwang is passionate about fostering strong, trust-based relationships with his patients and their families, aiming to support the overall health and development of children in underserved communities.
In his personal time, Dr. Hwang enjoys watching television, practicing meditation, and reading, activities that contribute to his balanced and mindful approach to both life and medicine.
Sarah Rahman, PA-C, Pediatric Physician Assistant
Sarah Rahman, PA-C is a certified physician assistant specializing in pediatrics at Urban Health Plan in the Bronx, New York. She earned her undergraduate degree from Wesleyan University in 2016 and completed her Master of Science in Physician Assistant Studies (MSPAS) at DeSales University in 2021.
Ms. Rahman’s clinical interests include preventive care, health education, and addressing health disparities in underserved communities. She is passionate about building strong relationships with her patients and their families, ensuring that all children receive compassionate, high-quality healthcare. Ms. Rahman is fluent in multiple languages, including English, Spanish, and Bengali, and is proud to serve the pediatric community of the South Bronx.
Dr. Patricia A. Anderson, MD, is a board-certified pediatrician with over 30 years of experience providing compassionate, comprehensive care to children and families in the Bronx, New York. She earned her medical degree from New York Medical College in 1991 and completed her pediatric residency at Montefiore Medical Center/Albert Einstein College of Medicine.
Dr. Anderson has been recognized for her dedication to patient-centered care throughout her career. From 2009 to 2012, she received the National Committee for Quality Assurance’s Physician Practice Connections Recognition Program and Patient-Centered Medical Home Recognition Program awards.
