At Urban Health Plan, we are proud to be one of New York State’s largest Federally Qualified Health Centers. Since 1974, our mission has been to improve health outcomes and quality of life by delivering comprehensive, compassionate, and accessible care to the communities we serve.
In 2024 alone, we:
Cared for 89,000 patients across the Bronx, Central Harlem, and Corona, Queens
✔️ Provided over 420,000 visits, including 16,000 for uninsured patients
✔️ Delivered care through 32 sites, including school-based and mental health centers
✔️ Contributed $252 million to the local economy
✔️ Engaged more than 18,000 community members through outreach and education
