Urban Health Plan has been awarded three 2025 Community Health Quality Recognition (CHQR) badges from the Health Resources and Services Administration (HRSA): National Quality Leader, National Quality Leader Heart Health, and Advancing Health Information Technology (HIT) for Quality.
The CHQR badges are awarded annually, recognizing health center program awardees and look-alikes (LALs) that have made notable achievements in the areas of access, quality, health information technology, and health outcomes using the Uniform Data System (UDS) for the 2024 reporting period.

Urban Health Plan was recognized with the following badges:
- The Health Center Quality Leader, which places UHP among the top 10% of health centers nationwide for overall clinical quality performance
- The National Quality Leader in Heart Health, which is awarded to health centers that achieve the established metrics to ensure the best outcomes in preventing heart attacks and strokes through excellence in cardiovascular disease prevention and management.
- The Advancing HIT for Quality, which recognizes how UHP has embraced innovation and technology to improve care delivery and patient outcomes. From telehealth to data-driven quality improvement, this badge reflects how UHP is not only keeping pace but leading in how technology supports health equity.
