If you need medical care after business hours, please call 718-589-2440. Give the operator your name and tell them you are a patient at Urban Health Plan. The operator will contact the provider on call, and they will return your call as soon as possible. If the line is busy, please call (212) 517-1891. If you have a life-threatening emergency, call 911 for an ambulance.
