To see a specialist at Plaza San Juan, you’ll need a referral from your Urban Health Plan provider.
This helps us make sure you get the right care at the right time.
If you’re already a UHP patient, talk to your provider at your next visit. They will help decide what specialty services are best for you and send the referral to our team.
