 Friday, April, 3rd, 2026:
• All Urban Health Plan locations are CLOSED
• The Virtual Care Center (Telehealth Services) will be OPEN 8AM-12PM 

Saturday, April, 4th, 2026:
• UHP Locations: El Nuevo San Juan (8 am – 5 pm); Plaza del Sol (7 am -1 pm); Vida Guidance (8 am – 2 pm)
• The Virtual Care Center (Telehealth Services) will be OPEN 8AM-12PM 

 Sunday, April 5th, 2026:
• All Urban Health Plan locations and Telehealth Services are CLOSED

UHP will resume regular hours on Monday, April 6, 2026

Should you need medical care while the center is closed, please call (718) 589-2440. 

Give the operator your name and tell them that you are a patient of Urban Health Plan. The operator will contact the provider on call, who will return your call as soon as possible. If the line is busy, please call (212) 517-1891.
