All Urban Health Plan locations and the Virtual Care Center (Telehealth Services) will be closed in observance of Good Friday on April 7th and Easter on April 9th.

On Satiurday, April 8th, the following health centers will be open:

The Virtual Care Center (Telehealth Services) will be open from 8 am to 8 pm.

El Nuevo San Juan Health Center at 1065 Southern Boulevard, Bronx, NY, will be open 8 am to 5 pm.

Plaza del Sol Family Health Center at 37-12 108th Street, Corona, NY 11368, will be open from 7:30 am to 1 pm.

If you are a patient and should need medical attention during this time, please call (718) 589-2440. Kindly provide the operator your name and tell them that you are a patient of Urban Health Plan.
