Bronx, NY — Urban Health Plan was recognized as a Health Center Quality Leader with a gold tier award from Health Resources and Services Administration (HRSA). The gold tier designates that Urban Health Plan is among the top 10% of health centers achieving the best overall clinical quality measures. Locally it is one of four health centers citywide and one of eight health centers statewide to receive the gold tier award.
The award is part of the Community Health Quality Recognition (CHQR) that presents badges to health centers that have made notable quality improvement achievements in the areas of access, quality, health equity, and health information technology for the most recent 2020 reporting period.

An Advancing Health Information Technology (HIT) for Quality award, which recognizes health centers for meeting all criteria to optimize HIT services that advance telehealth, patient engagement, interoperability, and collection of social determinants of health to increase access to care and advance quality of care.
The Patient Centered Medical Home Recognition (PCMH) award, which recognizes health centers with PCMH recognition in one or more delivery sites.

Health Center Quality Leaders achieve the best overall clinical quality measure (CQM) performance among all health centers. They are recognized in the following tiers: Gold (top 10%), Silver (top 11-20%), or Bronze top (21-30%).
