If you need medical care after hours, UHP provides on-call service on weekdays, weekends and holidays. To reach a provider, please call 718-589-2440. Give the operator your name and tell them that you are a patient at Urban Health Plan. The operator will contact the provider on call who will return your call as soon as possible. If the line is busy, please call (212) 517-1891. If you have a life-threatening emergency, call 911 for an ambulance.
