Services provided by the Health Information Management department: ​​

• Process medical records requests for patients, lawyers, doctor offices, ACS and HMOs (only with Authorization form)
• Request medical records from other health care facilities (only with Authorization form)
• Fax the authorization form to the Health Information Management department at (718) 650-8904.
