The Health Information Place at Urban Health Plan provides free assistance to patients and community members seeking support with health insurance. Our team can help you apply for health insurance, renew your coverage, and understand how your plan works. We also help people with problems related to their insurance, medical bills, or coverage. All services are free.
Health Insurance Enrollment Assistance
Urban Health Plan’s Health Information Place provides free help with health insurance enrollment through the New York State of Health Marketplace.
Individuals and families may qualify for no-cost or low-cost coverage, including Medicaid, the Essential Plan, Child Health Plus, Qualified Health Plans, and Emergency Medicaid. Our certified enrollers also provide education on the Affordable Care Act and help you understand your options and choose a health plan.
Documents Needed for Insurance Enrollment
In most cases, you will need the following:
-Identification
- New York State photo ID or U.S. Passport
–Proof of Immigration Status (if applicable)
- U.S. Passport
- Permanent Resident Card
- Work Authorization Permit
–Proof of Income
- Copy of your income tax return, or
- Four recent consecutive pay stubs (if paid weekly), or
- Two recent consecutive pay stubs (if paid bi-weekly)
