For over 50 years, Urban Health Plan (UHP) has served as a beacon of hope in communities that are too often defined by limited access to healthcare, investment, and opportunities. Founded in the South Bronx in 1974, UHP has grown into one of New York’s largest and most respected federally qualified health centers, now serving more than 89,000 patients annually across the Bronx, Queens, and Central Harlem.
This year, our Annual Gala pays tribute to our founder and true visionary, Dr. Richard Izquierdo, whose legacy continues to shape our mission of building healthier, stronger communities.
The evening will be hosted by Jeanine Ramirez, an award-winning journalist and five-time Emmy nominee. It will bring together more than 400 healthcare leaders, community advocates, business partners, and friends of Urban Health Plan for a night of celebration, connection, and impact.

The Dr. Richard Izquierdo Lifetime Achievement Award is the highest honor presented by Urban Health Plan.
It commemorates the extraordinary vision and legacy of our founder, Dr. Richard Izquierdo, whose life’s work transformed health care in the South Bronx and beyond. Doc believed deeply that health care is a right, not a privilege, and he dedicated his entire career to ensuring that families who were too often overlooked had access to the care, dignity, and respect they deserved.
This award is reserved for individuals whose life’s work embodies that same passion and commitment. Recipients of the Dr. Richard Izquierdo Lifetime Achievement Award have not only excelled in their chosen fields but have also dedicated decades to advancing health equity, uplifting underserved communities, and creating opportunities for others to thrive. Their leadership has had a sustained, generational impact, touching lives, shaping institutions, and inspiring future leaders to carry the torch forward.
In honoring these distinguished individuals, Urban Health Plan affirms that Doc’s spirit lives on. The award serves as both a recognition of extraordinary service and a reminder that when one person devotes their life to justice and community, the effects can ripple for generations.
The Community Hero Award honors individuals whose extraordinary service, leadership, and dedication have made a profound impact on the neighborhoods Urban Health Plan serves. This award celebrates those who go above and beyond to advance health equity, uplift families, and strengthen the social fabric of the communities served by Urban Health Plan.
Recipients embody the spirit of the Urban Health Plan’s mission to improve the quality of life for the people we serve. Whether through advocacy, direct service, or community leadership, honorees demonstrate a commitment to dignity, justice, and opportunity for all. Urban Health Plan Community Hero Award commemorates not only their personal achievements, but also their role in building healthier, stronger, and more resilient communities.
The Urban Health Plan Corporate Award recognizes companies and corporate partners whose leadership, generosity, and commitment to social responsibility have made a transformative impact on the communities Urban Health Plan serves. This award celebrates organizations that go beyond business success to invest in health, equity, and opportunity, standing as true partners in creating a stronger, healthier New York.
The Urban Health Plan Partner of Choice Award recognizes an organization or individual whose strategic partnership with Urban Health Plan has been essential in advancing our mission and strengthening the patient experience. This award is given to an entity or individual that goes beyond traditional collaboration, working side by side with UHP to break down barriers to care, create innovative programs, and expand access to vital services for underserved communities.
Recipients of the Urban Health Plan Partner of Choice Award exemplify what it means to add value to the patient journey. Through their expertise, commitment, and dedication, they help UHP deliver health care that is not only comprehensive and affordable but also rooted in dignity, trust, and community connection.
The Urban Health Plan Up-and-Coming Healthcare Leader Award honors a graduate of the Dr. Richard Izquierdo Health & Science Charter School who exemplifies the vision, determination, and spirit of our founder. The school was established with a vision of cultivating a local pipeline of leaders who would carry forward a legacy of service by becoming healthcare professionals dedicated to their communities. This award celebrates a rising leader whose academic and professional path reflects a clear commitment to healthcare and community service.
Recipients of the Urban Health Plan Up-and-Coming Healthcare Leader Award demonstrate not only promise in their chosen health fields, whether as doctors, nurses, researchers, or allied health professionals, but also a profound sense of responsibility to give back. They represent the future healthcare professionals envisioned: a new generation of caregivers and advocates equipped to transform lives, uplift communities, and lead with compassion and integrity.
