Friday, November 24 – All Urban Health Plan Centers are closed.

Telehealth Services are available from 8 am to 12 noon.

Normal business hours will resume on Saturday, Nov. 25, 2023 at El Nuevo San Juan, Vida Guidance Center and Plaza del Sol Family Health Center.

All other sites will resume normal business hours on Monday, Nov. 27, 2023

Should you need medical care while the center is closed, please call (718) 589-2440. 

Give the operator your name and tell them that you are a patient of Urban Health Plan. The operator will contact the provider on call, who will return your call as soon as possible. If the line is busy, please call (212) 517-1891.
