Please be advised of the holiday schedule changes for Urban Health Plan’s centers and services. 

Should you need medical care while the center is closed, call (718) 589-2440. The Virtual Care Center/Telehealth is available on July 4 from 8 am to 12 pm.

Give the operator your name and tell them that you are a patient of Urban Health Plan. The operator will contact the provider on call, who will return your call as soon as possible. If the line is busy,  call (212) 517-1891.

Urban Health Plan will resume its regular schedule on July 5, 2024.
