  • Friday, Dec. 24: Closed
  • Saturday, Dec. 25: Closed
  • Thursday, Dec. 30- 8 am to 3 pm
  • Friday, Dec. 31: Closed
  • Saturday, Jan. 1: Closed

If you need medical care on any of the above dates, please call (718) 589-2440, give the operator your name and tell them you are a patient of Urban Health Plan. The operator will contact the provider on call who will return your call as soon as possible. If the line is busy, please call (212) 517-1891. If you have a life-threatening emergency, call 911 for an ambulance.
