Join us in congratulating Paloma Izquierdo-Hernandez, our President, CEO, and Board Chair of the National Association of Community Health Centers (NACHC), who was named to City & State’s Bronx Power 100 list for the fifth consecutive year! This honor highlights Paloma’s exceptional leadership and profound impact across the Bronx, Central Harlem, Corona, Queens, and beyond!
Under Paloma’s direction, Urban Health Plan has grown into one of New York State’s largest community health systems, expanding to 12 federally qualified health centers, 12 school-based health centers, and two mental health sites, serving nearly 90,000 patients annually.
Paloma’s work goes beyond ensuring access to compassionate, high-quality primary care: She’s driven strategic growth in social programs tackling food access, housing, education, and job readiness.
