Thanking a Bronx Business for Bringing Healthy Food Options to the Community!

Urban Health Plan and the New York City Department of Health's Shop Healthy program (pictured below, right to left) presented a proclamation from Bronx Borough President Ruben Diaz, Jr. to Andy Silverman (center), owner of Sol's Pharmacy in the Bronx. Andy worked with Urban Health Plan's nutrition department to join the Shop Healthy program, which works with local businesses to bring healthy food options to the community. 

Boricua Community Health Center Opens its Doors to the Community

April 6, 2015 - Bronx, NY - Bronx residents living in the Morrisania area will have a new community health center providing quality healthcare to the whole family!  Urban Health Plan is opening the doors of its newest clinical site, Boricua Community Health Center.   

Boricua Community Health Center is a 6,000 sq ft, state-of-the-art facility located at 3209 Third Avenue, Bronx, NY 10451.  This Urban Health Plan site is capable of serving an additional 10,000 Bronx residents with quality services which include: primary care for all ages, eye care, foot care, psychology, nutrition, social services, health insurance enrollment, health education and immunizations.

Hours of operation are Monday throught Friday: 9:00am - 6:00pm and Saturday: 9:00am - 1:00pm.  For more information about Boricua Community Health Center and its services, please call (718) 860-5555.
