The MICHC Worker is a bridge between providers of health, social and community services and the underserved/ hard to reach populations within the community. They identify pregnant women, post-partum women, including those who are uninsured, underinsured, are not receiving prenatal care, health or other community services. MICHC helps to get women into prenatal care, other health care services, and connect them to community supports.
MICHC workers also provide important referrals, coordination and follow-up to needed services, as well as advocacy and service coordination. Additional services provided include support during pregnancies, postpartum transitions from hospital to home, breastfeeding support, and infant care education. The Maternal Infant Community Health Collaborative targets the Bronx. Clients are identified through outreach and through referrals. Our MICHC workers are residents of the community. MICHC workers speak both English and Spanish.
