The Bronx is New York City’s northernmost borough, located northeast of Manhattan. It has been Urban Health Plan’s home since 1974. It is home to such city landmarks as Yankee Stadium, the New York Botanical Gardens and the Bronx Zoo.
Through our sites in the South Bronx, Urban Health Plan serves the residents of Mott Haven, Morrisania, Hunts Point, Longwood, Third Avenue and Soundview.
Queens is New York City’s most populous and ethnically diverse borough. It is home to CitiField, home of the New York Mets, the United States Tennis Association Billie Jean King National Tennis Center in Flushing Meadows-Corona Park, the Queens Museum and the Museum of the Moving Image
Urban Health Plan’s Plaza del Sol Family Health Center is located in Corona, Queens, which is home to a large Latin American immigrant community with people from Colombia, Ecuador, Mexico and the Dominican Republic.
Harlem, in northern Manhattan, is home to our Citicares Community Health Center.

In addition to being close to wonderful Bronx neighborhoods, UHP is centrally located near the suburbs of New York, New Jersey and Connecticut, and other New York City boroughs, like Manhattan, Brooklyn and Queens counties.
Close to the Bronx is Manhattan, often considered the “core” of the Big Apple, which is home to trendy and upscale neighborhoods such as the Upper East Side, Upper West Side, Harlem and Washington Heights, exciting entertainment and nightlife, numerous museums and cultural institutions and historic landmarks. It will also be home to the transformational Hudson Yards development, a mix of residential, retail and park space.
Queens and Brooklyn offers a mix of suburban and urban residential communities. Residents of both boroughs often closely identify with their neighborhood, such as Corona, Jackson Heights, Long Island City or Astoria in Queens and Williamsburg, Boerum Hill, Brooklyn Heights and Park Slope in Brooklyn, rather than with the borough or city. Both boroughs are a patchwork of unique neighborhoods, each with its own distinct identity. Several of these neighborhoods are home to a diverse mix of ethnicities and cultures.
East of Queens and Brooklyn are Nassau and Suffolk counties, which make up Long Island. Like New Jersey, Long Island is known for its miles of coastlines bordering Long Island Sound to the North and the Atlantic Ocean to the South. Its upscale communities, excellent school districts, and beautiful beaches make Long Island a popular and scenic place to live.
Travel a few miles north from Urban Health Plan and you will find yourself near the historic Hudson River towns of Westchester County. The area’s beauty, the historic attractions and the excellent schools make Westchester a wonderful place to live and raise children.
Further east of Westchester County is Connecticut, a southern New England state that’s a mix of coastal cities and rural areas dotted with small towns. Known for its picturesque coastline along Long Island Sound, Connecticut
A few miles west of our main site is New Jersey, accessible by the George Washington Bridge and known for its proximity to New York City and its wonderful beaches. New Jersey’s crown jewel is the 127 miles of Atlantic Ocean coastline that runs from Sandy Hook to Cape May. Along the state’s shore towns are beautiful barrier islands, majestic lighthouses, scenic views and New Jersey’s famous boardwalks.
